Business Permit Procedure
  • Secure Business Permit Application Form at the Business One-Stop Shop (B.O.S.S.).
  • Completely fill out all entries on the application form
  • Secure the following REQUIREMENTS FOR BUSINESS PERMIT.
  1. Attach all required supporting documents to the properly filled out application form then submit to B.O.S.S.
  2. When everything is in order, taxes, fees and charges will be assessed by the Business License and Permit Division (BLPD).
  3. Assessment shall be paid to the Office of the City Treasurer.
  4. The Mayor’s Permit will be released at BLPD on a date specified on the claim stub to be provided by Office of the City Treasurer.

REQUIRED SUPPORTING DOCUMENTS TO BE SUBMITTED TO BLPD:
1. Isulat ang mga kaukulang impormasyon sa Application Form for Business.
2. Barangay Business Clearance para sa kasalukuyang taon na galing sa barangay kung saan itatayo ang negosyo. Original copy (for presentation only) and 2 photocopies to be given to BLPD & CPDCO

  • Photocopy of Latest Real Property Tax Receipt/Payment or RPT clearance from CTO (for registered owner/ lessor of
  • Contract of Lease/Award Notice/Authorization Letter/Certification (if renting)
  • Tax Declaration – for Lot / Building / Stall Rentals
  • Professional Tax Receipt – for exercise of profession (photocopy)
  • Affidavit/Undertaking on non-operations of games – for internet café
  • Grant of Authorities from Congress/PAGCOR/ Sangguniang Panlungsod Franchise – for amusement entities (carnivals, fairs and amusement parks, cockpit arenas, bingo parlors and outlets, pool and billiard halls, theaters and cinemas, night clubs and similar businesses) (photocopy)
  • CCTV Certification – for banks, malls, money changers, pawnshops, supermarkets, and other similar establishments
  • Original or Photocopy of Community Tax Certificate (CTC)/ Cedula (Cedula for Business or Corporate Cedula)
  • Requirements for Zoning Certification:
    • Tax Declaration
    • Any of the following applicable forms of ownership (photocopy):
      • Owner’s Duplicate Copy (TCT)
      • Contract of Lease duly notarized
      • Contract to Sell duly notarized
      • Deed of Absolute Sale duly notarize
      • Deed of Donation and/or Special Power of Attorney duly notarized
      • Market Enterprise (CEED) stamp - if within the new/old public markets
      • Authorization Letter and/or Notice of Award
      • Sworn Statement as Heir of the property
    • Neighborhood’s Consent nearby, minimum of 4 periphery units/Homeowner’s Clearance/Certification - if within residential approved subdivision
    • LHSD Clearance/Certification - if within the Northville Subdivision
  • Requirements for LBOD Clearance:
    • Services agreement (for security and janitorial services)
    • Affidavit of Undertaking (FM-CSFP-LBOD-04) – if on-going construction
    • Photocopy of Certificate of Occupancy (from the Owner of the building)
  • Requirements for Sanitary Permit:
    • For night clubs/entertainment establishments: (1)License to work – for entertainers/ models; (2)Results of the Cervical Urethral Smear from the Social Hygiene Clinic of GRO's
    • For Water Refilling Station:
      • Certificate of Certified Water Operator Course
      • Sanitary Plan
      • Physical/Chemical Test- every 6 months raw and product
      • Sanitary Engineer’s Report
      • Bacteriological Test TFC and HPC - product every month-raw every six months
    • For memorial park, funeral services and crematorium - Operational Permit/Clearance from CHD-3 (DOH), Barangay
    • For Industrial Establishment - Design of Water Treatment Plant facilities
    • For Poultry and Piggery - Barangay Resolution
  • Requirement for FSIS/FSIC:
    • Photocopy of Tax Declaration (for buildings except single family dwelling/ residential)
REQUIRED SUPPORTING DOCUMENTS TO BE SUBMITTED TO BLPD:

5. OTHER SUPPORTING DOCUMENTS:
  • For non-BIR registered taxpayers – indicate the Gross Sales / Receipts for the Latest Taxable Period/s on the Application Form for Business
  • For BIR Registered Taxpayers – any of the following: (Bring original copy for presentation/ verification)
    • - Income Tax Returns of the Preceding Year (photocopy)
    • - VAT or Percentage Tax Returns (Monthly / Quarterly) – Preceding Year (photocopy)
    • - Affidavit of Gross Sales / Receipts / Certification of breakdown of sales of each branch (if consolidated ITR)
  • For non-operation of businesses – Affidavit of Non-Operation for the Latest Taxable Period/s
  • For Banks- Notarized Joint statement of Annual Income (signed by a designated Officer of the Head Office and by the Branch Manager)
  • Photocopy of Latest Real Property Tax Receipt/Payment or RPT clearance from CTO (for registered owner/ lessor of the property)
  • Professional Tax Receipt – for exercise for profession (photocopy)
  • Grant of Authorities from Congress/PAGCOR/ Sangguniang Panlungsod Franchise – for amusement entities (carnivals, fairs and amusement parks, cockpit arenas, bingo parlors and outlets, pool and billiard halls, theaters and cinemas, night clubs and similar businesses) (photocopy)
  • PCAB Contractor’s License – For contractor, sub-contractors, specialty contractors engaging in the construction agency (photocopy)
  • BSP Proof of Registration (received Application Form at the BSP or Certification of Registration) – for pawnshops, foreign exchange dealers, money changers and remittance agents
  • Original or Photocopy of Community Tax Certificate (CTC)/ Cedula (Cedula for Business or Corporate Cedula)
  • Requirements for Sanitary Permit:

      • Chest x-ray result
      • Urine exam result
      • Stool exam result
    • For Water Refilling Station
      • -Bacteriological Test- product every month-raw every six months
      • -Physical/Chemical Test- every 6 months raw and product
  • Requirements for FSIS/FSIC:
    • Latest Fire Safety Inspection Certificate or Affidavit of loss, if lost
    • Photocopy of Tax Declaration (for buildings except single family dwelling/ residential)
